This post will be based in Milton Keynes.
To support young people to achieve good outcomes in key areas of their lives by ensuring that their comprehensive needs are incorporated in Pathway Plans and co-ordinating arrangements necessary to achieve successful transitions to independence.

Must have experience of working within a Looked After Children's or Young People's Team.
We require that that all social workers are fully qualified with a Degree in Social Work, DipSw, CQSW, CSS or equivalent social work qualification. We also require that you are GSCC registered and hold an enhanced CRB check obtained within the last 12 months.
