Description
This post leads health and social care commissioning for older people and adults with a physical disability (including Long Term Conditions), within the joint (health and social care) commissioning team. The post holder will work with Operational Managers across the health and social care system, including the independent and voluntary sector and other stakeholders to ensure that the Joint commissioning strategy for older people and adults with a physical disability (including long term conditions) is implemented.

The post will ensure that the strategic vision and social care economy continues to reflect the needs of older people, adults with a physical disability and their carers. The post holder will develop outcome based service specifications for all commissioned services (internal and external) and ensure a robust monitoring process is in place for the area of Older People and Physical Disabilities (including Long Term Conditions). As well as developing and reviewing local plans / joint plans and strategies in line with Government guidance, and national and local priorities, the post-holder will develop strategies the Primary Care Trust and Council to jointly commission services.

The post holder will line manage the OP/PD commissioning team including the LTC & Older People/Physical Disability Joint Commissioners.

Requirements
Relevant professional / management qualification
Should have or be prepared to develop critical appraisal skills.
Experience of speaking in public meetings.
Experience of working in both health and social services.
Will have direct experience of working with elected representatives and / or Health Authorities.

We require that all Social Workers are fully qualified with a Degree in Social Work, DipSW, CQSW, CSS or equivalent social work qualification.

We also require that you are GSCC registered and hold an enhanced CRB check obtained within the last 12 months.
